Care Responsibilities: As a Support Broker, I support people with both developmental and physical disabilities, including traumatic brain injury, the frail elder, and varying degrees of mental illness. My role varies on each team based on the client need, but my primary responsibilities are: following MA guidelines, completing CIP recertification documents, which includes a review/assessment of the Long Term Functional Care Screen to determine functional eligibility, facilitating and fostering professional, positive communication within the team, thoughtful, productive and outcome-based planning, paperwork and documentation, offering choices and encouraging independence, resource navigation, appropriation of funding, and ensuring the person’s health and safety needs are met within the parameters of our support system.
As a Team Leader, I am responsible for the following: facilitate monthly team meetings with a small group of co-workers, review CIP recertification documents for accuracy before submitting to Dane County, prepare and offer feedback on annual performance reviews for my co-workers or help with disciplinary plans, 610 reports for the agency, and being available for other duties and tasks as assigned by my Executive Director.
